PRECISE-DAPT Score

PRECISE-DAPT score är ett score som ska prediktera out-of-hospital blödning hos PCI-behandlade patienter som medicineras med DAPT: Datan kommer från PRECISE-DAPT-studien som analyserade ca 15000 PCI-patienter 1, instrumentet validerades i bland annat PLATO kohorten men är inte prospektivt testat i en RCT. Scoren ska användas vid hemgång vid det initiala vårdtillfälle för att stödja beslutet om DAPT ska rekommenderas som vanligt (i 12 månader), eller kortare (3-6 månader) vid ökad blödningsrisk, eller längre (12-24 månader) vid mindre blödningsrisk.

Användningen av PRECISE-DAPT-Score för att bedöma blödningsrisken diskuteras i 2023 ESC ACS Guidelines  2.) när det gäller att förkorta DAPT-behandlingstiden hos PCI-patienter med AKS och misstänkt hög blödningsrisk, 2023 ESC ACS Guidelines lägger dock mer vikt på ARC-HBR score. Även ESC NSTE-ACS 2020 diskuterade användningen av PRECISE-DAPT (‘The PRECISE-DAPT score may be used to guide and inform decision making on DAPT duration with a modest predictive value for major bleeding. The value in improving patient outcomes remains unclear.’).

Scoren beräknas utifrån 5 parametrar:

  1. ålder
  2. Kreatinine clearance
  3. Hemoglobin
  4. LPK
  5. Tidigare spontan blödning

En online-räknare hittar du på Precisedaptscore.org (använd punkt istället för kommatecken. Notera att Hb i scoret anges i g/dl, i Sverige ofta g/L. Dividera då Hb t.ex. 140 g/L till 14.0 g/dl för att använda i score)

Tolkning:

  • Score ≥25 poäng: Hög blödningsrisk -> förkorta DAPT (<12 månader).
  • Score <25 poäng: Standard-DAPT (12 månader) eller längre (-24).

(Förväxla ej med som är gjort för att användas 12 månader efter okomplicerat DAPT för frågan om DAPT ska förlängas.)
